This sounds a lot like Mondrian/Rietveld or Google Code's review tool. Check them out, either to use them or to steal good ideas from them:

I'm not too in love with Rietveld's interface, Mondrian is much nicer to use, and while Google Code's review interface is very nice, it's only available after comitting (although there's now support to review a proposed merge of a branch into another, by filing a ticket).

There's also the "gcl" tool used by the Chromium folks, but that's a bit more involved (although very easy to use once you get into it).
